CKMB

What it is (overview)

The CKMB test (Creatine Kinase–MB) is a blood test that measures the level of the MB isoenzyme of creatine kinase (CK) in your bloodstream. Creatine kinase is an enzyme found in muscle cells. The CK-MB form is found mostly in the heart muscle, so it can rise when the heart is injured.

When heart muscle cells are damaged—such as during a heart attack (myocardial infarction)—CK-MB can leak into the blood. In general, higher CK-MB levels suggest recent heart muscle damage. Doctors often interpret CK-MB together with other tests and your symptoms, because CK-MB can also rise with certain non-heart problems (for example, major skeletal muscle injury).

How results are typically understood: a normal or low CK-MB level makes significant recent heart muscle injury less likely (especially if repeated tests remain normal). A raised CK-MB level may indicate heart damage and is often checked in a series (repeat measurements over several hours) to see if levels are rising or falling, which can help estimate timing and track recovery. Many emergency departments now rely primarily on troponin for diagnosing heart attack because it is more specific, but CK-MB may still be used in certain situations, such as assessing possible re-infarction (a second heart attack shortly after the first) or when troponin testing is not available.

When & why it's usually done

CK-MB is most often ordered in emergency care when someone has symptoms that could be due to a heart attack or other causes of heart damage. It may be used alongside an ECG (electrocardiogram), troponin, and other cardiac enzyme tests to support a rapid medical diagnosis.

Your clinician may order a CKMB test if you have symptoms such as:

  • Chest pain, pressure, tightness, or discomfort (especially if it spreads to the arm, jaw, neck, or back)
  • Shortness of breath
  • Sweating, nausea, lightheadedness, or fainting
  • Unexplained fatigue or weakness, particularly in people at higher risk of heart disease
  • Symptoms occurring after heavy exertion, surgery, or a period of low oxygen/blood flow

It may also be ordered to:

  • Help evaluate suspected myocardial infarction when the timing of symptoms is unclear
  • Monitor for possible repeat heart injury after a recent heart attack or heart procedure (because CK-MB can return toward normal sooner than troponin)
  • Assess heart muscle stress or injury after certain cardiac events or treatments

Risk factors that may increase concern for a heart-related cause (and therefore prompt testing) include older age, smoking, diabetes, high blood pressure, high cholesterol, obesity, family history of early heart disease, and known coronary artery disease—key issues tied to overall heart health.

  • Myocarditis (inflammation of the heart muscle)
  • Cardiac trauma (e.g., injury to the chest that affects the heart)

  • Rhabdomyolysis (severe muscle breakdown)
