What it is (overview)

The Direct (Conjugated) Bilirubin test is a common blood test that measures the amount of direct (conjugated) bilirubin in your bloodstream. Bilirubin is a yellow pigment made when your body breaks down old red blood cells. Your liver normally processes bilirubin so it can be removed from the body.

There are two main forms of bilirubin measured in lab testing:

  • Unconjugated (indirect) bilirubin: not yet processed by the liver; not water-soluble.
  • Conjugated (direct) bilirubin: has been processed by the liver (conjugated) and is water-soluble, so it can be carried into bile and eliminated through the intestines.

In plain language, this test helps show whether your liver is properly processing bilirubin and whether bile is flowing normally out of the liver. When direct bilirubin levels are elevated, it often suggests that bilirubin is being conjugated but is not moving out of the liver as it should—commonly due to liver disease, inflammation (such as hepatitis), or bile duct obstruction (cholestasis).

Direct bilirubin is usually interpreted along with total bilirubin, indirect bilirubin (calculated), and other liver function tests (such as ALT, AST, alkaline phosphatase, and GGT). Higher bilirubin can contribute to jaundice (yellowing of the skin and eyes), dark urine, and light-colored stools.

When & why it's usually done

Your clinician may order a direct (conjugated) bilirubin test as part of a liver panel or bilirubin test to evaluate symptoms, monitor known conditions, or investigate abnormal lab results.

It’s commonly done if you have symptoms that may suggest jaundice or problems with the liver or bile ducts, including:

  • Yellowing of the skin or eyes (jaundice)
  • Dark urine or pale/clay-colored stools
  • Itching (often seen with bile flow problems)
  • Right upper abdominal pain, nausea, vomiting, loss of appetite
  • Fatigue, fever, or flu-like symptoms that may occur with hepatitis

This test is also ordered to help evaluate or monitor:

  • Abnormal results on routine liver function blood work
  • Suspected bile duct obstruction (for example from gallstones, inflammation, or tumors)
  • Known or suspected liver disease (fatty liver disease, cirrhosis, alcoholic liver disease)
  • Medication-related or toxin-related liver injury (some drugs can impair bile flow or liver processing)
  • Ongoing monitoring of chronic liver conditions or response to treatment
  • Evaluation of certain blood disorders; while many hemolytic causes raise indirect bilirubin, bilirubin patterns (direct vs. indirect) can help narrow the cause when anemia or red blood cell breakdown (e.g., hemolytic anemia) is suspected

Because bilirubin levels can rise for different reasons, separating direct (conjugated) from indirect bilirubin helps clinicians determine whether the issue is more likely due to bile flow blockage/cholestasis, liver cell injury, or increased red blood cell breakdown.

  • Hepatitis (viral hepatitis A, B, or C; toxic or autoimmune hepatitis)
  • Cholestasis (impaired bile flow)
  • Bile duct obstruction (e.g., gallstones, biliary strictures)
  • Cholangitis (infection/inflammation of the bile ducts)
  • Primary biliary cholangitis (PBC)
  • Primary sclerosing cholangitis (PSC)
  • Cirrhosis
  • Fatty liver disease (including metabolic dysfunction–associated steatotic liver disease, MASLD)
  • Alcohol-related liver disease
  • Drug-induced liver injury
  • Pancreatic or biliary tumors causing obstruction (e.g., pancreatic cancer, cholangiocarcinoma)
  • Hemolytic anemia (typically more associated with indirect bilirubin, but included in differential evaluation of elevated bilirubin)
