Eosinophils
What it is (overview)
The Eosinophils test is a blood test that measures the number of eosinophils in your bloodstream. Eosinophils are a type of white blood cell that play an important role in the body’s immune response, especially in conditions involving allergies, asthma, certain parasite infections, and other causes of inflammation.
Your result may be reported as an absolute eosinophil count (AEC) (the actual number of eosinophils per microliter of blood) and/or as a percentage of your total white blood cells (often as part of a complete blood count, CBC, with differential). In general:
Higher-than-normal eosinophils (eosinophilia) can suggest that your immune system is reacting to an allergic trigger (like seasonal allergies or medication), airway inflammation (often seen in some types of asthma), a parasitic infection, or other inflammatory/immune conditions. The degree of elevation and your symptoms help determine how concerning it is and what follow-up testing is needed.
Lower-than-normal eosinophils (eosinopenia) are usually not a major concern by themselves and can occur with stress responses, acute illness, or from certain medications (especially corticosteroids). Your clinician interprets low values in the context of your overall health and other lab results.
When & why it's usually done
Clinicians commonly order an eosinophils test when they need more information about possible allergic disease, asthma control, infection, or ongoing inflammation. It may be ordered as a stand-alone lab or more often as part of a CBC with differential.
This diagnostic test may be recommended if you have symptoms such as:
Wheezing, coughing, shortness of breath, or recurrent asthma flare-ups; persistent nasal congestion, sneezing, or sinus symptoms consistent with allergic rhinitis; itchy skin, hives, or eczema-like rashes; unexplained fever or fatigue; abdominal pain, diarrhea, or weight loss (particularly after travel or exposure risks for parasites); or persistent swelling of lymph nodes.
It’s also often used when there is concern for:
Allergy-related inflammation (for example, evaluating an “allergy test” workup alongside IgE testing or skin testing), suspected parasite infection (especially after travel to areas where parasitic diseases are common), monitoring known asthma with an eosinophilic component (eosinophilic asthma), assessing reactions to medications, or investigating unexplained abnormalities on routine blood work.
Because eosinophil levels can change over time, your provider may repeat the test to monitor trends—especially when symptoms persist, treatment is started (such as inhaled steroids or allergy medications), or when evaluating chronic inflammation.
