What it is (overview)

The Indirect (Unconjugated) Bilirubin test is a blood test that estimates how much unconjugated bilirubin is circulating in your bloodstream. Bilirubin is a yellow pigment made when old red blood cells break down. At first, bilirubin is “unconjugated” (also called “indirect”), meaning it has not yet been processed by the liver. Your liver normally converts unconjugated bilirubin into “conjugated” (direct) bilirubin so it can be eliminated from the body in bile.

In most laboratories, indirect bilirubin is calculated from a standard bilirubin panel: Indirect bilirubin = Total bilirubin − Direct (conjugated) bilirubin. Doctors interpret indirect bilirubin alongside total bilirubin, direct bilirubin, and often other liver function tests (such as ALT, AST, alkaline phosphatase, and GGT) to understand the likely cause of an elevated bilirubin level.

In plain language, results generally suggest the following:

• Higher indirect (unconjugated) bilirubin may mean your body is producing bilirubin faster than the liver can process it (for example, from increased red blood cell breakdown), or that the liver has difficulty “conjugating” bilirubin (converting it to the direct form). This pattern can contribute to jaundice—yellowing of the skin and eyes—especially noticeable in the whites of the eyes.

• Normal or low indirect bilirubin generally suggests bilirubin production and liver processing are in balance. If you still have jaundice symptoms, your clinician may look for other causes (including elevated direct bilirubin due to bile flow blockage).

When & why it's usually done

Your clinician may order an indirect bilirubin test as part of a bilirubin panel during a routine health screening or when you have symptoms that suggest jaundice, anemia, or liver-related problems. It is commonly used to help determine whether jaundice is more likely due to increased red blood cell breakdown, a liver processing issue, or another condition affecting bile flow.

This test is often ordered if you have:

• Signs of jaundice: yellowing of the skin/eyes, dark urine, pale or clay-colored stools, or generalized itching (pruritus).

• Symptoms of anemia or hemolysis (red blood cell breakdown): fatigue, weakness, shortness of breath, rapid heart rate, dizziness, or unusual paleness. The test can support evaluation for hemolytic anemia when paired with a complete blood count (CBC), reticulocyte count, LDH, and haptoglobin.

• Abdominal symptoms that can relate to liver or spleen issues: right upper abdominal discomfort, nausea, poor appetite, or an enlarged spleen (splenomegaly) noted on exam.

• Concerns about liver function: abnormal liver enzymes, known liver disease, or monitoring of liver health when taking certain medications.

• Newborn jaundice: bilirubin testing in babies helps clinicians assess jaundice severity and guide care (although neonatal evaluation follows age-specific guidelines and may involve additional measurements).

• Family or personal history of benign bilirubin conditions (such as Gilbert syndrome) or previous episodes of unexplained mild jaundice, especially during illness, fasting, dehydration, stress, or intense exercise.

Because indirect bilirubin is one piece of a larger diagnostic picture, doctors usually interpret it together with symptoms, physical exam findings, and related diagnostic tests to identify the underlying cause.

  • Hemolytic anemia (including autoimmune hemolytic anemia)
  • Inherited red blood cell disorders (e.g., sickle cell disease, thalassemia, hereditary spherocytosis)
  • Gilbert syndrome (common, usually harmless reduced bilirubin conjugation)
  • Crigler–Najjar syndrome (rare, severe impaired bilirubin conjugation)
  • G6PD deficiency (hemolysis triggered by certain foods, infections, or medicines)
  • Hemolytic transfusion reaction (red blood cell breakdown after incompatible transfusion)
  • Resorption of a large hematoma (breakdown of pooled blood after significant bruising/bleeding)
  • Neonatal jaundice (physiologic or pathologic causes, evaluated in context)
  • Liver disease affecting bilirubin handling (e.g., hepatitis or cirrhosis—often assessed with the full liver function panel and direct bilirubin)
