What it is (overview)
Mean Platelet Volume (MPV) is a routine blood test result that reports the average size of your platelets (tiny blood cells that help stop bleeding by forming clots). MPV is typically calculated by an automated blood analyzer as part of a complete blood count (CBC) and is interpreted alongside the platelet count and other CBC values.
Platelets are made in the bone marrow and released into the bloodstream. In general, larger platelets are often younger platelets and may be more active. That means MPV can give clues about platelet production and overall bone marrow function—for example, whether the body is making platelets quickly to replace ones that are being used up or destroyed.
In plain language, MPV helps answer: “Are my platelets mostly small/older or large/younger?” A higher MPV can suggest increased platelet turnover (your body producing and releasing larger, younger platelets), while a lower MPV can suggest reduced platelet production or smaller platelets. Importantly, MPV by itself does not diagnose a specific condition. Doctors interpret MPV together with your platelet count, symptoms, medical history, and sometimes a peripheral blood smear to evaluate blood health and possible clotting disorders or bleeding tendencies.

MPV is most often reviewed when you have a CBC, especially if your platelet count is abnormal or you have symptoms that could be related to bleeding or clotting. Symptoms or risk of clotting such as unexplained swelling or pain in a leg (possible DVT), chest pain or shortness of breath (possible pulmonary embolism), or a history of heart attack or stroke—especially when combined with other blood tests and risk factors.
Abnormal platelet count found on routine testing: Low platelets (thrombocytopenia) may prompt a closer look at MPV to help distinguish decreased platelet production vs. increased destruction/consumption. High platelets (thrombocytosis) may be evaluated with MPV to support assessment of reactive causes (like inflammation) versus bone marrow-related causes.
Monitoring known conditions affecting hematology and bone marrow function, including after infections, inflammatory conditions, autoimmune disease flares, major surgery, or certain medications that can affect platelets.
Follow-up testing when there is concern for a platelet disorder: MPV may be reviewed along with platelet distribution width (PDW), other CBC indices, and sometimes specialized tests depending on the situation.
