What it is (overview)

A Red Blood Cell (RBC) Count is a common blood test that measures how many red blood cells are present in a specific volume of blood, typically reported as the number of cells per microliter (mcL). Red blood cells are made in the bone marrow and contain hemoglobin, the protein that helps your blood carry oxygen from the lungs to the rest of the body and return carbon dioxide back to the lungs. Because of this role in oxygen transport, the RBC count is an important part of a broader health assessment.

This test is usually performed as part of a Complete Blood Count (CBC), a standard hematology panel that also evaluates hemoglobin, hematocrit, and other blood components. In plain language, an RBC count that is lower than expected often suggests you may not have enough red blood cells to carry oxygen efficiently (a common reason is anemia). An RBC count that is higher than expected may indicate your blood has more red blood cells than usual, which can happen with dehydration (less plasma fluid makes the count appear higher) or conditions that increase red blood cell production. When & why it's usually done

Doctors often order an RBC count (usually within a complete blood count) when symptoms or medical history suggest problems with oxygen delivery, blood loss, hydration, or blood cell production. It may be used to help diagnose anemia, evaluate dehydration, or assess potential bone marrow or hematology concerns.

Common reasons to check an RBC count include:

Symptoms that may suggest anemia or reduced oxygen transport: ongoing fatigue, weakness, shortness of breath with exertion, dizziness or lightheadedness, headaches, pale skin, rapid heartbeat, or cold hands/feet.

Symptoms that may suggest high red blood cell levels or thicker blood: headaches, blurred vision, dizziness, facial flushing, itching (especially after a hot shower), or unexplained high blood pressure.

Possible blood loss or nutritional deficiency: heavy menstrual periods, gastrointestinal bleeding (such as black/tarry stools), recent surgery or trauma, low iron intake, or known iron deficiency.

Dehydration or fluid status concerns: vomiting, diarrhea, inadequate fluid intake, or conditions/medications that affect body water balance.

Chronic medical conditions and risk factors: kidney disease (affects erythropoietin, a hormone that stimulates RBC production), chronic lung disease, sleep apnea, smoking, or living at high altitude (all may trigger increased RBC production).

Monitoring and follow-up: tracking response to treatment for anemia (iron, B12, folate), monitoring known blood disorders, or routine screening as part of an annual physical or general wellness blood test.
