Throat Swab Culture & Sensitivity

What it is (overview)

A Throat Swab Culture & Sensitivity (often called a throat culture or sore throat swab) is a laboratory test that checks for bacteria causing throat infections, most importantly Group A Streptococcus (GAS), the main cause of strep throat. A clinician gently swabs the back of your throat and tonsils to collect a sample. The sample is then placed on special media in the lab to see what germs grow (the culture).

If bacteria grow, the lab identifies the organism and may perform antibiotic sensitivity (also called susceptibility) testing. Sensitivity testing helps determine which antibiotics are most likely to work against the bacteria found and which may not be effective (antibiotic resistance).

What the results mean in plain language:

Negative/No significant growth: No disease-causing bacteria were found on culture. This makes bacterial pharyngitis less likely and supports other causes such as a viral sore throat (which typically does not need antibiotics).

Positive for Group A Strep: This supports a diagnosis of strep throat. Treating confirmed strep can shorten symptom duration, reduce spread to others, and help prevent complications (such as rheumatic fever in certain settings).

Positive for other bacteria: Less commonly, other organisms may be identified. Your clinician uses the result along with symptoms and exam findings to decide whether treatment is needed.

Sensitivity (if reported): The report may list antibiotics as “susceptible/sensitive,” “intermediate,” or “resistant.” “Susceptible” means the antibiotic is expected to work well; “resistant” means it likely will not.

When & why it's usually done

This test is usually ordered when symptoms and exam findings suggest a bacterial throat infection, especially strep throat, or when a rapid test is negative but suspicion remains. It helps avoid unnecessary antibiotics for viral illness while ensuring appropriate treatment when bacteria are the cause.

Your clinician may recommend a throat swab culture & sensitivity if you have:

Symptoms suggestive of strep throat or tonsillitis such as a sudden severe sore throat, pain with swallowing, fever, red/swollen tonsils (sometimes with white patches or exudate), tender swollen lymph nodes in the neck, or absence of cough (cough is more common with viral infections).

Persistent or worsening sore throat lasting several days, or symptoms that return soon after improving.

Recurrent episodes of sore throat/tonsillitis, where identifying the organism can guide targeted therapy.

Close contact exposure to someone with confirmed strep throat (for example, within a household or classroom), especially if you develop symptoms.

Higher-risk situations where confirming the cause matters (for example, outbreaks in schools/dormitories, or people with conditions where complications may be more concerning). Your clinician may also culture to evaluate for other less common bacterial causes if clinically indicated.

Because a throat culture can take longer than a rapid antigen test (often 24–48 hours or more depending on the lab), it is commonly used as a confirmatory test when rapid testing is negative but the likelihood of strep remains significant (particularly in children and adolescents).

  • Bacterial pharyngitis (non–Group A Streptococcus and other bacteria, when clinically suspected)
  • Tonsillitis (including bacterial tonsillitis)
  • Peritonsillar infection/early peritonsillar abscess (when throat infection is severe; evaluation is clinical, culture may support)
  • Scarlet fever (strep infection associated with a characteristic rash)
  • Recurrent streptococcal infections and possible carrier state (in selected cases)
